NH CARE Program

The NH CARE Program is designed to help people living with HIV stay healthy, connected to care, and supported. Our goal is to reduce barriers to medical care by helping with medications, health coverage, and essential support services. This program is confidential, client-centered, and focused on your long-term health.

What is NH CARE & Who Qualifies?

Program Overview

The NH CARE Program is part of the Ryan White HIV/AIDS Program, a federal program that helps people living with HIV access medical care and support services.

In New Hampshire, the Department of Health and Human Services (NH DHHS) receives Ryan White Part B funding to support HIV services across the state and partners with community-based organizations like MVAP to provide care locally. MVAP works directly with clients to enroll in the NH CARE Program and connect them to the care and support they need. Through this program, eligible individuals can receive help with HIV medications, medical care, and case management support.

NH CARE Program Services

The NH CARE Program offers a range of support services designed to help people living with HIV access care, maintain stability, and meet essential daily needs. Our team works with each client individually to understand their situation and connect them with the services that will be most helpful.

Case Management


Our Case Managers provide personalized support to help you navigate services and stay connected to care. This may include assistance with applications, referrals, and connecting you to resources such as health insurance, housing support, Social Security benefits, and other community services.

Prescription & Coverage Assistance (HDAP/ADAP)

The NH CARE Program includes the HIV Drug Assistance Program (HDAP), also known as ADAP, which helps eligible individuals access HIV medications and health coverage. This may include help paying for prescriptions, insurance premiums, medical visits, co-pays, and lab testing.

Medical Care & Support

MVAP helps you find and stay connected to HIV care providers, primary care, and other health services. This may include support with finding providers for mental health care, dental care, substance use treatment, and other ongoing health needs, as well as help with scheduling appointments and navigating care.

Housing Assistance

Stable housing is an important part of staying healthy. The NH CARE Program may be able to provide emergency rent or utility assistance to help prevent homelessness and support housing stability when eligible.



Transportation Services

Getting to appointments and services is an important part of staying connected to care. This may include help with transportation to medical visits and other essential services, including rideshare or taxi support when needed.

Food and Nutrition Support

Access to food and nutrition plays an important role in overall health. MVAP offers on-site food pantries with perishable and non-perishable items, as well as household and personal hygiene supplies. This may also include medically prescribed nutrition services when appropriate.

In order to be eligible for the NH CARE Program, you must meet these three requirements:

  • Be living with HIV/AIDS
  • Be a New Hampshire resident
  • Have an annual gross household income at or under 500% of the Federal Poverty Level

When you meet with a Case Manager, we will ask for a few documents to confirm eligibility, including:

  • Proof of income (if applicable)
  • Proof of New Hampshire residency
  • Proof of insurance (if you currently have insurance)
  • Proof of your HIV diagnosis

Please note that income and insurance are not required to enroll in the program. If you do not currently have income or insurance, we can complete a self-attestation form during your appointment.

Our team will guide you through the enrollment process and help determine which services and programs you may qualify for.

What Happens Next?

Once you connect with us:

  1. A Case Manager will schedule an appointment with you.
  2. We will review your eligibility for the NH CARE Program.
  3. We will help enroll you in services and connect you with any additional support you may need, such as medical care, medications, insurance coverage, housing assistance, or other support services.

Our goal is to make the process simple, supportive, and focused on your needs.

Even if you do not qualify for NH CARE, we may still be able to connect you to other resources, including pharmaceutical patient assistance programs, community services, and other programs that may not have income limits. We are here to help make sure you have access to the care, medications, and support you need.
